  The Advance Notice of Injury form must be completed by the Coach and submitted to CDYSL.
  
  Note: You have 90 days from date of injury to submit the claim form. For claims to be eligible for coverage you must seek medical attention within 60 days from the date of injury.

Coaches Licensing

• Approved Waiver Requests for 2011-12: (pdf) This list contains the names of coaches who have applied and been approved for a 1-year waiver to complete a qualified coaching license course
• Expired Waiver Requests: (pdf) This list contains the names of CDYSL Coaches whose 1-year temporary waivers have expired as of 8/31/11. These coaches are no longer eligible to appear as a head coach on CDYSL rosters until proof that they have taken a qualifying coaches coaches is provided to the league registrar
